Problem uploading images

When I try to post an image into a comment on my forum, I can open the 'Insert Image' window, ask it to 'Choose File' as normal, but when I then press the 'Insert image' button, all I get is a blank pinkish window (as per screenshot below), and no image ever comes at all:
