Sometime images disappear.

This has happened before in the magazine.

I upload an image and then it's gone.



When I quote the post, and find the image URL, past into a browser, I get a 404 error.  

The I upload again.

What causes this? Can it be fixed?

Do you have a test sub forum inside your magazine? I have to try to reproduce this bug.
